> [!NOTE]
> This feature is optional. If you don't see **Forms** in your Admin, your site may not use it or your role may not have access. Ask your agency if you'd like to use it.

You'll build a reusable form, add it to a page, and send a test response. A form is saved separately from its page, so the same form can be used in several places.

## Build the form

1. Open **Forms** and choose **Create new Form**. Enter a **Title** that your team will recognize, such as Contact enquiry.
2. Select **Add Field** and choose the kind of question you need.

3. Enter the field's **Label**. This is the question visitors see. Add a **Placeholder** if an example would help. Tick **Required** only when you need an answer before the form can be sent.

4. Add the remaining questions. Keep the form short. For choices, use clear option labels. Leave each question's **ID (lowercase, uppercase, numbers, no special characters)** unchanged after people start submitting answers. It identifies the answer in stored responses and exports.
5. Set **Submit Button Title**, for example Send enquiry.
6. Choose **Confirmation Type**. **Message** shows a thank-you message; **Redirect** sends the visitor to another page or address. Write a useful **Confirmation Message** or choose the redirect destination.
7. Select **Save**. Updating a saved form can affect every page using it.

| Field or choice | What it does | Tip |
| ---------------------- | -------------------------------------------------------- | ----------------------------------------------------------------- |
| **Text** | A short written answer. | Use it for a name or a short question. |
| **Email** | An email address. | Use this type when you need to reply to the visitor. |
| **Textarea** | A longer written answer. | Use it for an enquiry message. |
| **Select** / **Radio** | A choice from listed options. | Use familiar words and avoid overlapping choices. |
| **Checkbox** | A tick-box answer. | Make its meaning clear without relying on the visitor ticking it. |
| **Date** / **Number** | A date or numeric answer. | Explain the expected information in the label. |
| **File Upload** | Lets visitors attach a file when enabled. | Ask your agency about allowed files and size limits. |
| **Note** | Explanatory text, rather than a question. | Use it for instructions or a privacy-policy link. |
| **Required** | Prevents sending the form with that question unanswered. | Ask only for information you need. |
| **Advanced Settings** | Holds additional options for the selected field. | Leave unfamiliar options as set by your agency. |

## Add it to a page

1. Open the page in **Pages** and place the cursor where the form should appear in **Content**.
2. Open the blocks menu at the left of the toolbar and choose **Form**.
3. In **Select a form**, choose the form you saved.

4. Use **Publish changes** when the page is ready. Open the public page and fill in a test answer.

5. Send the test. Check the confirmation and find the response in **Form Submissions**. If notification emails are set up, also check their destination inbox.

## Check afterwards

Check every question, required-field message and confirmation. Try the page on a phone. Confirm that the response appears in [submissions](https://docs.systhema.app/hu/editor-guide/forms/submissions.md); a visible form alone does not prove that responses reach you.
